Our medical records request process ensures your records are safely and confidentially maintained while providing you ready access when you need them. There are a few ways you can request copies of your medical records.

How to Request Your Medical Records

To obtain a copy of your medical records from CaroMont Regional Medical Center or any CaroMont Medical Group doctor's office, contact the HIM/Medical Records Department at 704.834.2111 or visit the Customer Service counter located on Level B (off the visitor parking deck) at CaroMont Regional Medical Center Monday through Friday, 8:30 a.m. to 5:00 p.m.

Patients may complete the Patient Medical Records Request Form and send it to HIM/Medical Records at the address or fax number listed below.

Third-party requesters such as attorneys or other authorized representatives may complete the Authorized Representative Request Form and send it to HIM/Medical Records at the address or fax number listed below.

Some medical records are also available through the MyChart portal, a secure online tool that lets CaroMont Health patients view portions of their electronic medical records. Request an account at mychart.caromonthealth.org for free access to test results, appointment information and visit summaries. Need something more?
